When admitted to hospital, you may look forward to returning to your own home, to recuperate in the place where you feel most comfortable and relaxed.
Our hospital to home service can enable you to return home after a stay in hospital. We can help you to prepare to settle back in to your familiar surroundings by visiting you in hospital to plan your return home.
To ensure a smooth transition home we will liaise with other health care professionals as required e.g. Physiotherapists, Occupational Therapists, District Nurses and GP’s.
Our care workers can provide personal care and ensure all other important jobs are taken care of from a lift home from the hospital to shopping and meal preparation to laundry and light housework.
