Choosing home care for yourself or a family member is an important decision. Our aim is to make it easier for you.

MF Care Management is a person centred service, unique to each individual. Our team of case managers ensures the care needs of each person are met through the implementation of an outcome-based care plan, needs and risk assessment. This is regularly reviewed to identify changes in care needs.

Case managers are responsible for specialist training identified in the care plan and to ensure that all our team members have the knowledge and skills to provide the care required.

Our services are flexible and can be arranged to meet you support needs and choices. Times of visits can range from 30 minutes to 24 hours a day.

We have experience in providing care for the following:

  • Complex care – both paediatric and adult
  • Long term health conditions e.g. MS, MND, Parkinson’s, Huntington’s
  • Cerebral palsy and epilepsy care management
  • Head and Spinal Cord injury
  • Dementia care and stroke rehabilitation support
  • Palliative care for those approaching end of life
  • People with learning difficulties and mental health needs
  • People with sensory loss and dual sensory loss

MF Care Management can be accessed in the following ways:

  • If your needs have been assessed by the Local Authority or Health Service.
  • If you have Self-Directed Support Funding and wish to purchase support
  • If you wish to make a private arrangement directly with ourselves

Personal Care

This service is accessible for people who may require assistance with personal care, washing, assistance with medication, cooking, getting ready for bed and getting up in the morning.

Medication Support

Are you worried about remembering which medication to take and when? Or is a relative starting to become forgetful about their medicaton? 

Our medication support service is there to help.

Respite Care

Looking after someone can make it hard to take a break. Our respite service provides the care you and your loved one need, whether it’s for a couple of hours, a couple of weeks or even longer.

Reablement

If you have been in hospital, or are  recovering  from  an  accident,  we’ll  provide  support  to  help  you  regain your  confidence  and independence.

Companionship Services

Our team can provide the support you need to carry on enjoying favourite or meaningful activities, from going to the theatre to visiting the dentist or doctor.

Domestic Support

Poor health can make it difficult to keep on top of your household chores – we can help lighten the load for you by helping with light domestic tasks around your home.

Meal Preparation

Food is an essential part of everyone’s health and wellbeing, whatever their age. We want to help our service users enjoy nutritious meals at home and out and about.

Nutrition Support

A regular well, balanced meals can make all the difference to a person’s quality of life. We explore with you at the beginning of our relationship your needs and priorities when it comes to food.

Hospital to Home

If you are returning from a stay in hospital it is important to have a smooth transition from hospital to home especially if you live alone. Our care workers can ensure all the important jobs are taken care of from shopping and meal preparation to an initial meet up and we can accompany you home.

Live-in Care

If you or a loved one have more complex needs, we can arrange for full­time, live-in carers, so that support and assistance are always on hand. We can also provide sleepover services, so that if you need help at night, there is someone close at hand.

End of Life Care

When one of your loved ones has a short time left to live, we can be there to help both you and them through this difficult and distressing time.
