Our hospital to home service can enable a seamless return home after a stay in hospital. We can help start the transition with a hospital visit to identify individual needs and preferences, as well as liaising with other health care professionals such as Physiotherapists or Occupational Health, District Nurses and GP’s.
Our care workers can provide personal care and ensure all other important jobs are taken care of from a lift home from the hospital to shopping and meal preparation to laundry and light housework.
